Prognostic Value of Normal Stress Myocardial Perfusion Imaging and Ventricular Function in Japanese Asymptomatic Patients With Type 2 Diabetes

چکیده
atients with type 2 diabetes mellitus (DM) have a higher risk of cardiovascular events and death than those without the disease.1–6 Coronary artery disease (CAD) in diabetic patients is frequently silent.7–9 To maximize the effective treatment of cardiovascular disease in patients with DM, it is important to objectively identify CAD in asymptomatic subjects in a noninvasive way as early as possible.4–6 Risk stratification is essential for the development of evidence-based strategies for improved patient care for those with DM.5,6 The evaluation of known or suspected CAD using ECGgated myocardial perfusion single-photon emission computed tomography (SPECT) imaging has been established for diagnosis and risk assessment by many precedent studies.10–14 The Japanese Assessment of Cardiac Events and Survival Studies by quantitative gated SPECT (J-ACCESS) is the first large-scale prognostic study using myocardial perfusion imaging (MPI) of an Asian population.15–17 Cardiac event rates associated with normal or low-risk myocardial perfusion SPECT imaging with 99 mTc-tetrofosmin have been shown.17 Following the J-ACCESS study, J-ACCESS-2 began focusing on DM.18 In general, patients with normal SPECT images have a lower risk of death and hard cardiac events, whereas in those with abnormal MPI, the mortality and hard cardiac

Clinical value and severity of myocardial perfusion defects in asymptomatic diabetic patients with negative or weakly positive exercise treadmill test
Objective: Although coronary artery disease (CAD) is the leading cause of death in type 2 diabetic patients, it is frequently asymptomatic. Myocardial perfusion imaging (MPI) is reported to show ischemia in a significant number of asymptomatic diabetic patients. We studied the prevalence and severity of myocardial perfusion defects in asymptomatic diabetic patients and its clinical impact. Meth...
متن کاملThe role of oral dipyridamole stress gated SPECT in assessing the response of the left ventricle to stress: Re-evaluation of an old method
Introduction: We aimed to assess whether the vasodilator effect of oral dipyridamole on the left ventricular systolic function in patients with suspected CAD is different from that of intravenous (IV) dipyridamole using Tc-99m MIBI myocardial perfusion gated SPECT. Methods: Eighty-nine patients (17 M / 72 W; 61 ± 10 years) were enrolled in this study. The patients underwent a dipyridamole stre...
متن کاملDipyridamole stress and rest gated 99mTc-sestamibi myocardial perfusion SPECT: left ventricular function indices and myocardial perfusion findings
Introduction: We investigated the difference in left ventricular ejection fraction (LVEF) and end-systolic volume(ESV) measured by gated myocardial perfusion SPECT (GSPECT) in the post-dipyridamole stress and rest periods, and compared the results with the perfusion patterns found in the conventional non-gated tomograms. Methods: 297 consecutive patients were studie...
متن کاملDetermination of normal ranges of regional and global phase parameters using gated myocardial perfusion imaging with Cedars-Sinai’s QGS software
Introduction: Myocardial perfusion imaging using gated SPECT and phase analysis is an effective tool in evaluation of mechanical dyssynchrony. The purpose of this study was to determine the normal ranges of global and regional phase parameters. Methods: A total of 100 patients with normal resting and stress electrocardiograms, low pretest likelihood for c...
متن کاملThe prognostic value of stress/rest gated myocardial perfusion SPECT in patients with known or suspected coronary artery disease
Introduction:Gated myocardial perfusion SPECT has diagnostic and prognostic values in coronary artery disease (CAD). We tried to determine prognostic values of the left ventricular perfusion & functional indices as well as transient left ventricular dilation (TID) derived from gated myocardial perfusion SPECT. Methods: 1820patients who underwent gated myo...
